This role represents an exciting opportunity for a passionate and dedicated Diabetes Educator to join our Community Health team for the provision of services that align with our strong focus on best practice clinical assessment, treatment and care.

The Diabetes Educator practices as a part of a multi-disciplinary team across all parts of the organisation – Community Health, Acute and Aged Care.

The role involves clinical management, preventative health and interventions for the Yea community, aged care residents and acute patients.

You will ensure our clients in all service areas have access to high quality care and provide internal committees with objective assessment reports on the quality of services being provided.
