YDMH is seeking applications for a Social Worker Grade 2, to join our Community Health team to delivery safe, high quality services to our residents, patients and community, across acute, residential aged care and community health.

Are you passionate about making a real difference in people’s lives? Our small, close-knit rural health service is looking for a compassionate Social Worker to support individuals and families across our community.

In this role, you’ll work closely with patients, families, and healthcare staff to provide psychosocial support, advocate for client needs, and connect people with services that promote health and wellbeing. You’ll also play a key part in community outreach and health promotion initiatives.
