This fixed term role will be the lead driver of the development, implementation, coordination and ongoing evaluation to the Lower Hume Mental Health Service to support and provide interventions for people experiencing mental health illnesses in Mitchell and Murrindindi Shires.

You will be the clinical lead for the program, developing and leading the service delivery model, while maintaining strong working relationships with a wide range of services essential to providing whole of person care, including developing referral pathways. 

The team will benefit from your leadership, strong focus on capacity building, highly developed service knowledge and service navigation expertise to support people of all ages experiencing mental health issues.

The role will be based at Yea and District Memorial Hospital and will require travel to other program sites.
