About the Role

The Inreach Support Coordinator is a part of a consortium led by Homeless Healthcare, with the aim to provide individualised, psychosocial and recovery support interventions, to people experiencing homelessness, who present to Royal Perth Hospital. The goal of these interventions is to enhance clients’ health and social wellbeing, housing, quality of life, maximise self-management, independence and community participation to help transition out of homelessness.

This is a full-time fixed-term position until 25 October 2025 in line with current funding.

Hours of work are Monday to Friday 8am - 4pm

Responsibilities include but are not limited to:

  • Client engagement and relationship building.
  • Case management and planning working collaboratively in a multi-disciplinary team, completing referrals and maintaining appropriate records.
  • Providing support and interventions within framework to address client's presenting needs to improve their quality of life.
  • Attend networking opportunities and community activities to promote service, to enable effective referrals and stay informed on services availability

What we’re looking for

The successful applicants will be required to meet the following criteria:

  • Have a relevant tertiary degree in a related discipline and/or relevant work experience (3- 5years) which demonstrates strengths in the criteria listed below. 
  • Demonstrated advanced understanding of complex issues facing people who experience chaotic life circumstances such as homelessness, AOD use, family/domestic violence, and mental health issues. 
  • Knowledge and understanding of diversity, gender and social inclusion, particularly Aboriginal culture. 
  • Advanced personal goal planning skills to work within an appropriate model to achieve client goals and improvement in quality-of-life outcomes. 
  • Ability to be reflective and open to continued learning.
  • Highly developed report writing using analytical reasoning. 
  • Advanced computer skills, proficient in Microsoft Office and client reporting systems. 
  • Demonstrated experience in building and maintaining professional relationships with clients and their wider natural supports with multiple needs and issues. 

 Specialist Criteria

  • Experience with homelessness and tri-morbidities. 

Required compliance documents:

  • Current WA drivers’ license
  • National Police Clearance Certificate (& International Criminal History Check, if applicable)
  • WA Working with Children Check

  • A base annual salary aligned with Level 4 of the Ruah Enterprise Agreement (and the SCHADS Award), ranging between $85,399 - $91,881 + 11.5% superannuation.
