In this role, you’ll work within Residential Aged Care settings, providing tailored, evidence-based interventions designed to support residents in living fuller, more independent lives. You’ll also contribute to our innovative Plena Care Pathway — an award-winning model of care that puts client goals at the centre, combining 1:1 therapy, care plan reviews, and active ageing group sessions to deliver the best possible outcomes.

What You'll Be Doing

• Conducting comprehensive functional assessments and delivering clinical recommendations
• Supporting residents with mobility, ADLs, falls prevention, pain management, and pressure care
• Advising on equipment and aids to enhance independence and safety
• Delivering both individualised and group-based therapy programs
• Collaborating within your scope of practice to ensure high-quality care and compliance
• Flexibly responding to the needs of residents, aged care facilities, and the wider team
