Registered Nurse

We are seeking an experienced Registered Nurse to join our TCP - coordination team. The ideal candidate will have:

  • Recent experience as a Transition Care Program (TCP) coordinator or in a complex care coordination role
  • Excellent communication skills, both written and verbal
  • Ability to undertake comprehensive assessments to:
  • Determine client eligibility for our program
  • Assist in developing client-centered, goal-oriented care plans
  • Ability to liaise with a broad range of stakeholders along with leading the multidisciplinary team
  • Strong organisational skills
  • Understanding of the new Aged Care Act and Aged Care Quality Standards

In this role you will coordinate and facilitate the transition of older people from hospital back into the community through assessment, care planning, referral and support. You will work closely with clients, families, care teams and other partners to enable positive outcomes and optimise quality of life.
