Are you passionate about empowering patients to manage their health and live better lives? Mansfield Hospital is seeking a dedicated and compassionate Chronic Disease Educational Health Nurse to join our dynamic team.

In this pivotal role, you’ll work closely with individuals living with chronic conditions—such as diabetes, heart disease, COPD, and more—delivering tailored education, support, and health management strategies. You’ll be instrumental in helping patients understand their conditions, improve their quality of life, and reduce hospital readmissions.

Be part of a multidisciplinary team to provide an educational and exercise program for consumers who have issues with their cardiac and respiratory health.

We are looking for someone with experience in

• Chronic disease health
• Excellent communicatoin and interpersonal skills
• Ability to work autonomously and as part of a team
• Leadership skills to supervise new graduate and students
• Excellent time management skills

services. It is also the responsibility of all staff to develop and maintain a working knowledge of the

National Safety and Quality Health Service Standards (NSQHSS) and Aged Care Quality Standards relevant to their position
