About the role:
Working in accordance with the objectives of Mansfield District Hospital, the Care Finder is a non-clinical role focused on supporting vulnerable senior members of our community to understand, access and connect with relevant supports in the community. The Care Finder sits in the Support at Home team and reports to the Support at Home Manager.
The Care Finder works independently to engage in outreach, develop referral pathways, liaise with other health/support services and provide support to clients. The Care Finder will work to identify barriers to service for vulnerable community members and aim to facilitate service delivery for these clients.
The Care Finder will act as a knowledge hub for community members, other health and support organisations and community groups.
Key Selection Criteria:
Essential:
- A minimum of 3 years’ experience working in a community-based aged care setting
- Relevant qualifications as Allied Health Assistant; Certificate III or IV Aged Care / Disability/ Leisure and Lifestyle
- Excellent written and verbal communication skills, including the ability to work with consumers of diverse backgrounds
- Excellent presentation skills and confidence to deliver evidence-based strategies to Senior Management, including sharing ideas and highlighting key areas of focus and improvement
Skills and Abilities:
- Good time management and organisational skills
- Proficient in using the Microsoft Office suite of programs
- Proficient in delivering reporting/ data requirements
- Effective written and verbal communication skills
- Demonstrated commitment to the organisation’s values.
