KnG Healthcare are at the forefront of this innovative model, working intimately with hospitals to facilitate seamless transitions from hospital discharge to home or onward to another care facility. Key Responsibilities:

  • Provide supervisory medical care to patients in the transitional phase, including assessment, treatment, and follow-up.
  • Collaborate with the multidisciplinary healthcare team to ensure coordinated and holistic patient care.
  • Maintain accurate and up-to-date medical records and documentation.
  • Support and supervise Nursing and allied health staff.
  • Participate in clinical meetings, case conferences, and quality improvement initiatives.
  • Be available for on-call duties and weekend shifts as required.

Requirements:

  • Medical degree from a recognised institution.
  • Full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Vocational registration trained GP (e.g.RACGP).
  • Demonstrated clinical competence and experience in a hospital or similar setting.
  • Strong interpersonal skills and ability to communicate effectively with patients, families, and colleagues.
  • Commitment to continuous professional development and quality improvement.
  • Compliance with all mandatory training, certifications, and regulatory requirements.
  • Vaccination against COVID-19 and influenza.
  • Australian Workers Rights.
