The Role:
We are seeking a qualified and compassionate Physiotherapist to join our Home and Community Care team in the North Metropolitan area of Perth.
In this mobile and autonomous role, you will provide physiotherapy services in the community and at Juniper TCP sites, using a reablement approach to support clients’ independence, mobility, and overall wellbeing.
This role can be offered as full time or part time (minimum three days per week), with no shift work, and the option of a partially hybrid working arrangement.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualised care plans
- Deliver tailored individual and group exercise programs to improve function and independence
- Provide equipment assessments and prescriptions to support client safety and autonomy
- Communicate effectively with clients, families, and other healthcare providers
- Maintain accurate clinical records and documentation
- Collaborate closely with multidisciplinary team members and Therapy Assistants
Key Requirements:
To thrive in this role, you will be a motivated and empathetic clinician who is passionate about making a positive impact in the lives of older adults. You’ll be confident working both independently and as part of a supportive team, with a strong focus on person-centred care.
- AHPRA Registration, current and full registration as a Physiotherapist
- Experience in aged care or community settings (desirable but not essential)
- Excellent interpersonal and communication skills – able to engage effectively with clients, families, and colleagues
- Strong organisational and clinical documentation skills with attention to detail and time management
- Demonstrated commitment to holistic and client-focused care
- Collaborative approach where you are able to work with multidisciplinary teams, including nurses, therapy assistants, and other allied health professionals
- Current Australian driver’s licence and reliable vehicle (essential) – you’ll be travelling throughout the North Metropolitan region
- Confidence using technology – comfortable with mobile devices, documentation software, and remote work tools
