The role:
We are currently seeking an Occupational Therapist to join our friendly and supportive Home and Community Care team whilst also serving our TCP facility in Karrinyup. Our Juniper Home and Community Care team provides OT services to assist our aged customers to live independently in their own homes. The TCP provides short-term care that seeks to optimize the functioning and independence of older adults after a hospital stay. The aim of the program is to provide older people with an option to improve their functional independence and enable them to return home or provide support to secure permanent aged residential care. In this rewarding role, you will be a valued member of our multidisciplinary team, delivering high-quality, resident-focused Occupational Therapy services. You will work closely with our residents and their families to conduct assessments, identify individual capabilities and preferences, and set realistic goals that help maintain or restore independence.
The successful candidate will be offered a permanent contract working 37.5 hours per week. Hours are Monday to Friday each week during normal business hours and split across both the TCP.
Key Responsibilities:
- Conduct resident assessments, determine individual capabilities/preferences, and set achievable goals.
- Support residents in achieving these goals while maximizing their functional capacity for daily activities.
- Prescribe aids and equipment as needed.
- Maintain effective communication with residents, their families, and the multidisciplinary team.
- Supervise and support therapy assistants.
- Address the concerns and requests of residents' families and other caregivers.
- Collaborate with the multidisciplinary team to ensure comprehensive care.
- Identify areas for improvement and use a continuous quality approach to problem-solving.
- Collaborative involvement in the TCP by delivering tailored goal focused interventions
- Work across a range of funding programs including Home Care Package, Commonwealth Home Support Program and Transition Care Program
Key Requirements:
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Occupational Therapist (essential).
- Previous experience as an OT in aged care (desirable).
- Proven ability to assess, plan, implement, and evaluate quality care.
- Empathy for elderly clients and the ability to work effectively with other health professionals and service providers.
- Excellent interpersonal and communication skills.
- Current Australian driver’s licence
