At Infinite Aged Care, the critical role of Care Manager is responsible for ensuring the effective and efficient delivery of care and clinical services to our residents whilst leading our care team.

Working closely with the Care Manager, Care Coordinators, Clinical/Care staff and supported by our Ashmore Facility Manager, you will work with limited direct supervision and be responsible for delivering resident-centric care in a timely and compassionate manner which enables residents to make choices and have control over their support needs.

Whilst leading and guiding the team, you will welcome new and prospective residents and their families to the facility, you will also be responsible for; generating care plans, conducting clinical assessments, monitor medication management, develop AN-AAC submissions, whilst ensuring that information is accurately recorded in accordance with all organisational and legislative requirements (AN-AAC), resident records and staff time recording.

You will also be responsible for management and monitoring the handling of comments and complaints in collaboration with the Facility Manager in a timely and resolution focused manner.

The ability to “think on your feet”, communicate, with, build rapport and maintain professional relationships with residents and co-workers is essential as is the desire to making the most of the personal and professional development opportunities we offer.

 About You

At Infinite Aged Care, we hire on attitude and aptitude, you do need to be aligned with our philosophy and values have strong communication skills, be a natural leader, be computer literate and have a commitment to resident wellbeing as well as

  • Minimum of 12 months experience as a Clinical Manager or similar leadership position.
  • A Bachelor of Nursing degree.
  • Have a valid AHPRA Registration.
  • Have experience in preparing and conducting clinical assessments.
  • Are comfortable and experienced in developing care plans.
  • Are familiar with AN-AAC and associated compliance requirements.
  • Are willing to work to meet the needs of the residents and facility.
  • Hold valid NDIS Workers Screening Clearance (or have the ability to get one)
