The Role

As Care Manager, you’ll play a pivotal role by delivering exceptional person-centred care 

management to our elderly clients in their own home. You’ll be more than just a case 

manager - you’ll be an advocate, problem-solver, and partner in helping our elderly

clients live fulfilling, independent lives.


What You’ll Be Doing

• Work closely with home care package recipients to design care plans that prioritise independence, well-being, and dignity.

• Collaborate with our Care Companions and internal teams to ensure the seamless delivery of high-quality care.

• Connect with clients, families, and stakeholders to foster trust, engagement, and exceptional outcomes.

• Identify risk factors, engage in a co-management approach to care planning ensuring appropriate referral of clients to specialised services.

• Ensure integrity of client related documentation and data entry, maintaining accurate and timely records in Age Up Health’s client care management systems


Who You Are

• You have a background in case management, aged care, or community services and are dedicated to making a difference.

• Experience as a Registered/Enrolled Nurse – Division 2 (preferred)

• Thrive in a fast-paced, agile environment and love problem-solving and taking initiative.

• Your ability to engage, advocate, and inspire shines through in everything you do.

• You believe in innovation and constantly seek better ways to deliver exceptional care.

• A valid Driver’s Licence and a reliable vehicle are required for this role. Regular travel within the East, South-East and Inner Metro region with occasional travel to the Northwest region. Travel is reimbursed.
