About the Role

Our HCP Care Partners, are integral to delivering tailored & trusted support to our clients, helping them to achieve their personal goals and getting to know them to ensure their health & wellbeing needs are taken care of. They are the client’s advocate within our dedicated team.

You will support with passion and commitment up to 50-60 clients, continuously assessing their health & wellbeing and come up with person-centred solutions in consultation with them and a diverse range of stakeholders to ensure high-quality care.

Beyond supporting our existing clients, HCP Care Partners are very important to meet prospective new clients, giving them a sense of relief that help is in sight and to build trust that they can count on us to focus on great solutions in partnership with them.

What You’ll Be Doing

  • Crafting personalized care plans tailoring your approach to each client’s individual needs, goals, and preferences.
  • Building strong client relationships by developing deep, meaningful connections with them and understanding their unique stories and what drives them.
  • Growing your client portfolio and managing your own client base demonstrating why we’re the best choice for homecare
  • Driving community impact by actively contributing to our reputation by delivering exceptional, person-centred care.

The role is based out of our Tullamarine office with WFH flexibility and travel to visit clients as required. 

About You

  • 3+ year of relevant experience in a similar role, helping you to hit the ground running.
  • Knowledge, understanding and experience in Aged care, Home or Community care
  • Passion for outstanding service with a genuine desire to go above and beyond in every interaction.
  • Strong communication skills with the ability to connect deeply with your clients and build lasting relationships.
  • An innovative mindset with the ability to think creatively and contribute ideas that set us apart.
