About the Role

The Team Leader will be responsible for the provision of quality care and services to participants in accordance with care plans, and to be in compliance with legislative requirements and government standards. This will include contribution to the planning, development and review of policies, guidelines, and service delivery processes.

In addition, the role of a Team Leader is to provide coaching, leadership and development to House Leaders and ensure best practise standards are maintained, leading to improved outcomes for the safety and wellbeing of participants.

The Team Leader will also continue to find new and develop existing stakeholder relationships with a view to continually improve the participant experience as well as expanding services to include more participants.

 

Key Responsibilities and Tasks

The key responsibilities of a Team Leader include leading and developing a team of House Leaders to engage with family members and stakeholders, and work directly with participants to ensure they achieve their goals and remain commented to the community.

 

Support Worker coaching and development

·       Contribute to a high performing, risk adverse and positive engagement culture aligned with the organisation’s CARE values

·       Lead, coach and develop a team of House Leaders; including onboarding, mentoring and coaching, performance management and reviews, training and WHS

·       Maintain regular, proactive engagement with House Leader, including a documented monthly 1:1 supervision, to assist in building strong engagement and relationships with House Leaders

 

Client management and care

·       Provide a delivery of care services that respect the rights of the participants, including maintaining confidentiality; and respecting the individuals’ values, customs, preferences as well as spiritual beliefs

·       Play a pivotal role in ensuring that participants receive high quality, culturally appropriate care, ensuring that their needs are appropriately identified and addressed.

·       Work cooperatively with other staff involved in the care and development of participants and to support is aligned to the participant’s plan

·       Maintain all relevant administrative and documentation requirements in accordance with company and NDIS standards including always ensuring the use of Salesforce, updating client files, notes, and meeting notes

 

Day-to-day operations

·       Establish and maintain positive rapport with stakeholders both internal and external

·       Oversee roster scheduling where needed, working closely alongside the Workforce Planning and Scheduling department

·       Manage conflict, provide coaching and identifying areas of skill development

·       Always ensuring the use of Salesforce, updating client files, notes, and meeting notes

·       Partner with the People and Culture team to attract and retain suitable candidates in a planned and proactive manner

 

WHS and risk management

·       Conducting on site audits ensuring medical and meal logs are maintained and compliant, the house is well maintained and safe, client notes and folders are up to date and accurate

·       Conduct monthly spot checks with all homes, inclusive of environmental, medication, administrative and maintenance.

·       Support and communicate the implementation of WHS principles within the residential placement

·       Complete near miss/hazards/injury register forms when appropriate

·       Adherence to the Complaints Management Process ensuring complaints are appropriately handled, communicated, closed and documented.

 

Business Development

·       Find/generate leads to fill SIL Vacancies

·       Build relationships with stakeholders who can refer participants to Connecting families

·       Maintain a BD database for your region

 

Relationship Management

·       Host/facilitate stakeholder meetings for participants

·       Continually look to improve the Connecting families experience for our participants

·       Recognise and respond to any complaint or feedback with a focus on continuous improvement

 

How Success is Measured What KPI’s for this role??

·       Monthly 1:1 supervision with support workers is conducted and documented

·       Intakes are managed

·       Site audits are conducted on the property and client files and are documented with no adverse findings

·       Understand and abide by all Connecting Families policies and procedures through completion of core mandatory training and refreshers

Position Criteria

Essential

·       Certificate 3 in Community Services, Youth Work, or Disability/Aged Care

·       Demonstrated experience in residential services, within a house leader/house coordinator capacity

·       Well-developed supervision skills and demonstrated experience in people leadership

·       A minimum of 5 years experience working with people living with a disability particularly in an accommodation setting

·       Experience in conflict management, de-escalation, incident management and prevention

·       Mentoring, mediation and conflict resolution skills with an ability to respond appropriately to crisis situations in an unsupervised setting

·       Demonstrated ability to provide client focused services with positive outcomes

·       Exceptional interpersonal relationship skills with the ability to manage and nurture key relationships with internal and external stakeholders

·       A current Working with Children Check

·       A current NDIS Screening Worker Check

·       A current police check conducted within 6 months

·       A valid Australian drivers licence with a reliable and comprehensively insured car

·       A current first aid certificate within 6 months of application
