The Opportunity
The transition to the National Disability Insurance Scheme (NDIS) has represented a significant shift in the way disability supports are provided to people with disability. However, the complexity of accessing and navigating the scheme can leave vulnerable families at risk of scheme exclusion or from optimising the funded disability supports they are entitled to receive.
This position is pivotal in providing much needed support to vulnerable families and children to access and participate effectively in the NDIS. The position covers LGAs of Baw Baw and Latrobe.
This is a newly established role dedicated to working with vulnerable families with disability and improving child and family services sector responses to these families.
Key Responsibilities
The Specialist Disability Practitioner role involves the provision of effective case management and support for children, young people and their families. This is achieved through the provision of case work, parenting strategies, tailored Family Action Plan and referral/networking to other service providers including child protection.
The role provides specialised in-home case management and engagement of vulnerable families, providing parenting skills, professional development, secondary consultation, and referrals.
About you
You will hold a minimum Diploma level qualification in Social Work, Psychology, Disability, Welfare or a related discipline along with experience in working with families in the health and welfare sector as well as NDIS.
You will have specialist knowledge and experience in working with vulnerable children and families from a diverse background and presenting with a variety of issues and needs, including resistance to engagement with professionals and an understanding of the issues confronting children and families involved in the child protection system.
