Who are we looking for?
The Relief Clinical Coordinator plays a pivotal role in ensuring the delivery of high-quality, person-centred care across multiple homes. This position is responsible for stepping in during periods of leave or absence, providing leadership and guidance to staff, and ensuring that clinical services are maintained at the highest standards. The Relief Clinical Coordinator will assist in maintaining a positive and supportive work environment while mentoring and coaching staff to foster a culture of excellence in care delivery. Additionally, when coverage at a home level is not required, the Relief Clinical Coordinator will assist in managing key projects, contributing to the continuous improvement of clinical services across Residential Aged Care Services.
What can you expect to be doing?
Clinical Support and Care Delivery:
- Provide clinical leadership and coordination across assigned homes, ensuring that person-centred care principles are adhered to.
- Oversee and assist in the development and implementation of care plans for residents, ensuring they meet individual needs and preferences.
- Ensure the safe and effective administration of medications and medical treatments in line with clinical policies and regulatory standards.
- Conduct regular assessments of resident care to monitor progress, identifying potential risks and implementing appropriate interventions.
Coaching and Mentoring:
- Provide coaching, mentoring, and support to nursing and care staff to enhance clinical knowledge, skills, and performance.
- Promote best practice in clinical care and ensure staff are up-to-date with current healthcare practices and industry standards.
- Assist in fostering a collaborative team environment that values ongoing learning, accountability, and quality improvement.
Leadership and Staff Supervision:
- Assume leadership responsibility for clinical and care staff in the absence of permanent coordinators or managers.
- Act as a role model for clinical excellence, professional conduct, and adherence to organisational values.
- Support team members in managing clinical challenges, providing guidance on decision-making, and encouraging the use of evidence-based practices.
- Maintain open communication with all levels of staff, encouraging feedback and promoting staff engagement.
Quality and Compliance:
- Monitor compliance with regulatory standards, policies, and procedures, ensuring that all clinical operations are aligned with legislative and accreditation requirements.
- Participate in internal audits and assessments to ensure the clinical care environment meets the highest standards of care.
- Collaborate with the Clinical Services Manager and other leadership personnel to implement improvements based on feedback, audits, and quality assessments.
Communication and Reporting:
- Provide timely and comprehensive reports regarding clinical operations, staffing, and resident care issues to the Clinical Services Manager.
- Act as a liaison between families, residents, and healthcare professionals, ensuring all parties are informed and involved in the care process.
- Address any concerns or incidents in a prompt, professional, and sensitive manner, ensuring that resolutions are communicated effectively
Project Assistance:
- Assist in key clinical projects when coverage at a home level is not required, including quality improvement initiatives, policy updates, or staff training programs.
- Work closely with senior leadership to identify, prioritise, and implement projects that enhance the overall efficiency and quality of clinical care across Residential Aged Care.
What do you need to bring?
Clinical Expertise: Demonstrated ability to manage complex care needs and provide clinical oversight. Minimum of 5 years’ experience in a clinical care setting, with at least 2 years in a leadership or coordination role.
Leadership Skills: Strong leadership and mentoring abilities, with a focus on team development, motivation, and professional growth.
Communication: Excellent written and verbal communication skills, with the ability to engage effectively with staff, residents, families, and external professionals.
Problem-Solving: Ability to identify challenges and develop practical, person-centred solutions.
Adaptability: Flexibility to work across multiple homes, adapting to different team dynamics and clinical needs.
Commitment to Quality Care: A strong commitment to promoting high-quality, evidence-based clinical care practices.
Qualifications: Registered Nurse (RN) with current AHPRA registration.
Experience in residential aged care or similar environments is highly desirable.
Knowledge of relevant aged care regulations, standards, and guidelines (e.g., Aged Care Quality Standards)
