Who are we looking for?

As a Case Manager, you will play a crucial role in providing intensive support to older adults experiencing homelessness or at risk of homelessness. You will work collaboratively with clients, service providers, and community agencies to develop and implement comprehensive case plans, addressing their complex needs and empowering them to achieve their goals.

What can you expect to be doing?

  • Deliver direct interventions tailored to the diverse needs of older adults experiencing homelessness.
  • Develop and maintain strong relationships with clients, building trust and rapport.
  • Conduct comprehensive assessments to identify client needs and develop individualised support plans.
  • Advocate for clients' rights and access to essential services.
  • Collaborate with internal and external stakeholders to coordinate care and maximise outcomes.
  • Provide crisis intervention and support as needed.
  • Maintain accurate and up-to-date client records.


What do you need to bring?

To be successful in this role, you will possess:

  • Relevant tertiary qualification in Human Services or equivalent (e.g., Social Work, Mental Health Nursing, Youth Work, Disability).
  • Significant experience working with marginalized and vulnerable populations, including older adults experiencing homelessness.
  • Demonstrated case management skills, including assessment, planning, and evaluation.
  • Strong understanding of the NDIS and Aged Care systems.
  • Excellent verbal and written communication skills.
  • Ability to build rapport and establish trust with clients.
  • Strong organisational and time management skills.
  • Commitment to working collaboratively with a diverse team.
