Who are we looking for?

We are seeking an energetic and highly motivated person who is passionate about providing holistic care to join our Transition Care Program team as a Clinical Coordinator. This role is a fast-paced position, centred around the assessment and implementation of rehabilitation plans for people returning to their homes after receiving hospital care.

You will find yourself working with our customers to create, implement & case manage up to 12-week rehabilitation plan by engaging Allied Health Practitioners & other health care professionals. This is an integral role which empowers our customers to recover in their own homes & ensures they will receive the highest level of care and support possible.

This position is best suited to Allied Health Professionals and Nurses.

What can you expect to be doing?

  • Meet with customers across Adelaide Metro to assess and create up to 12 week rehabilitation plans.
  • On-going case management of customers up to 12 weeks.
  • Engage and arrange the appropriate Allied Health Practitioners & Health Care Professionals for your customers.
  • Provide treatment to customers based on your clinical background and expertise as a part of rehabilitation plans.
  • Work collaboratively with AnglicareSA's broader Health & Wellness and Community Aged Care teams

What do you need to bring?

  • Current AHPRA registration or background
  • Demonstrated experience working as an Allied Health Professional or a Nurse in an environment subject to accreditation
  • Tertiary Qualification in an Allied Health profession or Nursing
  • Proven ability to be flexible, adaptable & able to work collaboratively within a multi-disciplinary team environment
  • Excellent communication skills, emotional intelligence, with the ability to build rapport with customers and stakeholders quickly
  • Experience in Aged Care desirable
