The Department
The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department.
Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.
Social Work provides a range of services including:
- Counselling
- Discharge Planning
- Crisis interventions
- Bereavement counselling
- Family support
- Care coordination
- Making Appropriate referrals
- Community liaison
Position Summary
A fantastic opportunity exists for an experienced Social worker to work in this new and exciting role within the Spinal and Rehabilitation Service at Caulfeld Hospital. You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in the NIDS, Hospital discharge planning, grief and loss counselling and staff leadership and support. You will possess strong assessment, case planning, report writing experience and have an understanding of discharge services which will be essential to this role. A knowledge of working with patients with a Spinal injury and patients needing long term Rehabilitation is cruical. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism, innovation and the ability to find creative solutions to challenges is highly regarded. You will be required to provide leadership to the Spinal Social work service as well as support designated Rehabilitation units at Caulfield Hospital in collaboration with the Social work Rehabilitation Team Lead. You will be an experienced clinician who is able to provide comprehensive psychosocial assessments, crisis intervention, adjustment counselling, therapy, case planning, family work, community liaison, networking and referral to external stakeholders and discharge planning. A key aspect of this role will also be to provide leadership, supervision and support to junior Social work clinicians. The successful applicant will need to contribute to quality improvement and research activities and provide staff and student supervision.
Experience and Qualifications Required
- Applicants must hold a Bachelor Degree in Social Work
- Applicants must have an understanding of the psychosocial issues associated with this patient cohort
- Substantial experience in working within a hospital setting will be essential to this role
Essential:
- It is vital that the successful candidate has experience in working within a hospital setting, has leadership experience and has understanding of working with an Electronic medical record.
- A high level knowledge of NDIS, Aged care and Hospital discharge planning processes is essential for this role. Please ensure you include a cover letter indivating the key skills outlined in this advertisement.
